Most is actually being able to handle Photoshopping.
[Tutorial] Creating a Windows 7 or Vista Visual Style - Themes and Visual Styles - WinMatrix (http://www.winmatrix.com/forums/index.php?/topic/14347-tutorial-creating-a-windows-7-or-vista-visual-style/)

This link should help you through it.

i found that tutorial a while back but didnt really help when it came to editing the start menu, since the custom start menus i see have an image and all the programs shifted over to the right by roughly 100ish pixels
